Help identifying 16613 insert

I recently purchased this 16613 bluesy and I am uncertain if the bezel insert is a service insert or the original insert. The watch is dated 2006/7 with serial D687xxx

Appreciate some help identifying the insert type and what the indicators are to determine its type.

I can see that some 16613 inserts have more of a flat top on the four - why is that?
