Watch breakout information form - Fedex

What is a watch breakout information form?

I have a watch being sent from Japan via Fed Ex. The watch ahs been held up until I fill out a watch breakout information form. What will happen when I do this? Will I now need to pay an import tax on this used watch?

Actually its in PDF form...

Anyway, itsa form that declares wether you are a 'business' or 'end user', and as you are an 'end user' you would have to give them your social security number and other info before they release it from customs...

I'm not sure if the shipping invoice was filled out correctly. I'm worried that Fed Ex will want me to pay an import tax.

FedEx will not require you to pay anything extra...

The Federal Customs Office will. It is Customs who are likely holding up the watch and you need to deal with them to make the proper declaration, pay customs duties and fees, then they will release the watch and FedEx can deliver it..

My bad. The form you are asking about, should have been sent by the seller to FEDEX. This form is NOT YOUR RESPONSIBILITY...

The watch was shipped by a family member. They did not fill out the forms correctly they only stated "watch", and checked that it was a gift. FedEx contacted me and asked to fill out the form.

You will get a bill in the mail in a couple months from FedEx import services most likely (my last was shipped via UPS so I got the bill from them)

What kind of watch are you importing? If its a Rolex it may never get to you.

If it isn't a Rolex expect to pay a couple % of its value.

Recently I receive a Panerai watch valued at $4000 from an individual in Hong Kong. I was required by Fed Ex to fill out the watch worksheet which was provided via a PDF email file.
The declared value was $2000 (50%) for the movement, $1200 (30%) for the case and $800 (20%) for the strap.
I was able to fax the worksheet back to Fed Ex the same day and the watch was delivered the following day.
The total of duty and taxes was $101.13 including a $25 customs fee that Fed Ex charged for acting as the customs broker.
The total of $101.13 was 2.5% of the value of the watch.
