When does a case tube need to be replaced?
Rolex Service seems to change them automatically with every overhaul. If you change the gaskets, do you only REALLY need to change the threaded tube if the threads are not stripped or cross-threaded?

No, as long as the crown & tube pass a water resistance test, they are good to go. I just recently acquired a Sub from from 1970 that still had the original crown & tube, I opted to change to a new crown & tube as I like my watches water tight.

The tubes are not all that expensive and yes at service they are normally changed better to be safe than sorry.
