Of course you are right, engine maintenance is expensive. At some point (maybe about 10 years ago?) I think ICAO changed some of the ETOPS rules governing how long an airplane is allowed to fly on only 1 engine to reach a diversion airport. Before that some routes required 3 or 4 engine planes.
