Icon5 Understanding "T<25" history, sequence

If someone could lay out this chronology (or point me to further reading), I'd really appreciate the help.

What was the issue?

When did this designation appear on Rolex dials?

Was there an overlap period, before and after?

Is is possible that one watch in a particular series would have "Swiss" only after another release that had "Swiss T<25"? (If so, how could that be?)

Was there a recall or similar effort around the transition time to get dials on watches in the field updated?

Were there any issues (real or perceived) w/ "T<25" watches, prior to the change over to Luminova?

Other answers to questions not asked to help w/ my education would be all the more valuable!
