Just kidding. Early in my career, I frequently first-assisted in surgery with an older guy who smoked throughout cases. The nurses would slip the lit cigarettes behind his mask. He'd take a drag and the exhaled smoke would rise in front of his eyes from the top of his mask.

In training, I recall big city surgeons' lounges with ash trays piled six inches high with cigarette butts.
