Watch Appraisal Criteria

Does anyone care to share their comprehensive appraisal of a watch for insurance purchases?

I realize model/serial/movement,etc., but I would appreciate seeing an example of a well written appraisal covering all features of the watch. (No names, address, value).

I searched forum, maybe not enough, for any previous post that had an actual example, or at least a bullet list of essentials for a comprehensive appraisal, but I didn't come across anything. Lots of conversations about insuring and claims, etc., but no one posted an actual appraisal.
I will call insurance co after holiday for their necessary criteria, also.
