He was probably acting snobby because your name was not on the warranty card (although its never okay to act snobbish to a customer). My advice would be to call other AD's and see if they will do one for you. Good Luck!

I Have purchased two Rolexes a Sub 116613LB & a Daytona 116503. I didn't buy them from a Rolex AD but as in your case the Warranty Cards were in other peoples names the Watches were brand new with all the factory seals in place. The Jeweller's I purchased from kindly valuated the two watches on their headed note paper & I had no trouble insuring them. Three years ago I purchased a Vintage Model & brought it over to the only Rolex AD in our City. There was no bother valuating the Watch but it did cost 100 Euros but they did a very professional job with photos included. So I would say to you look around for an AD that will do the job it will cost you a fee but if you want to insure your Watch you have no other choice

At the very least the AD should have explained why he wouldn't give the appraisal.

Appraisal
I have a watch from the 1950's that belonged to my aunt. No idea where she bought it. I went to a Rolex dealer in NYC and they refused to appraise it. How does one get an appraisal on a Rolex if you don't know which dealer it came from?

Independent jewelers who are licensed can often provide an appraisal.

I am currently having a watch serviced by Rolex NY and I just got off the phone and they are sending me an appraisal. Will post back when I get it.

Clearly the AD should not have been snooty. Unfortunately a lot are, it's just bad business.
On the other hand I can understand not wanting to appraise a watch without a bill of sale if you're not the original owner.

Call a local Jeweler. They should be able to refer you to an appraiser.
