If you sell through some of the well-known internet sites, then a packing slip or BoS is often generated automatically when the sale is processed. Otherwise, I'm sure you will find many example documents and templates if you search online

You can make your own with very little work. I prefer it that way - that way you get to decide what information is put on it (that they get to keep). Depending on what I am selling, I will have two copies labelled 1/2 and 2/2, with signatures.

Agreed - if it's for a straightforward personal sale, easy to just do one yourself
