It really depends on what size you’re looking for and what would fit your lady, but I think the smallest is 38mm in the Panerai Due line.

Look seriously at a Panerai Radiomir. Full Panerai DNA in a refined simple form. Various case sizes, looks amazing in gold.

How about a 1048? It is 40mm Luminor case with a crown guard and looks great on feminine wrists. First hand information.

The Panerai Luminor Due - 38 mm is a beautiful piece that would certainly look stunning on a lady's wrist!

My wife wears this 42 mm Radiomir quite easily...
So, I would say a 38 mm or a 42 mm Due can fit quite well.

I think the PAM 512 would be a good choice for a smaller wrist if you are willing to forego owning a Luminor model. For most ladies, no matter what Panerai you get (even a Luminor Due model) the watch will give you an oversized look. This is presumably what you are going for. I would be really careful though with a 44-47mm model. If the lugs are hanging off the sides of your wrist, the watch will just look foolish.

My wife loved her 48 and 49 equally. Great size and wonderful straps are available which go with all the handbags.
