Does the date change occur when the 12 hour hand hits midnight or the 24 hour hand (assuming the 24 hour hand is set to a different time zone than the local 12 hour hand)?

12 hour hand as that is used for date adjustment.

That's correct.
It is expected that when you wear it, the hour hand will always be set to where you are; therefore, it will always change at midnight at your location.
