Hi Parachrom! Looks like an old US Divers Mistral or Super Mistral!
I learned to dive with one of these "two hose" regs, more than 40 years ago!
I haven´t seen one of these in a long time! Thanks for showing.
Nice piece indeed as well as your SD. (I own a 1680 Sub).
