I actually procured it through Shreve, but it was shipped to me from schiffman. They called to confirm it was swiped and they filled out and took photocopies. I don't think I will have a problem at all. Thanks for the support. I haven't taken it off my wrist since.

Put the date. Keep the name blank. You only need to put your name when and if u send it for service, which will probably be in 5 years or more. By that time you might have flipped it.... or not....

regardless, keep the name field blank. Helps if u ever resale.
