I saw one on eBay the other day but I think it was $1500 and that was just the ceramic insert. The way Rolex is cracking down on "loose" parts I could see this being VERY expensive. Sorry I can't send you to someone specifically. I doubt that the one I saw on eBay was actually from Rolex.

For a couple grand you can get it... If your not willing to spend that you will not. To me I would not modify my watch this way but to each their own

I'm pretty sure if someone has one, they know they can get much more than $1k for it. Many have expressed interest in "sharking" their sub, few have been successful. If you find one for $1k, go for it but I doubt you will. Good luck.

I suppose your best luck would be at an EU Rolex service center. Unlike in the US, here you can order any parts you want. As long as you pay for it, you can buy any non-matching parts.
