How to find a Reference Number?

I have an old (1959) Caliber 2003. I know its serial number and the date of manufacture (I even have the original AP paperwork).
My crystal cracked and I need a new one.
One store asked me for the reference number. Can anyone tell me how to figure out the reference number to this watch?
Can anyone suggest a place to pick up a crystal?
